The Madlanga Commission of Inquiry has heard that six tender documents submitted by bidders to the Tshwane Metro Police Department went missing last year.
Suspended Chief Financial Officer, Gareth Mnisi, admitted there were major shortcomings in the document control process, saying an individual had been held accountable for the loss.
However, when pressed to identify the person, he said he could not recall.
Last week, Mnisi told the Commission it was not possible for tender documents to be manipulated, as they were kept under lock and key.
