3 tier penalty.
1) Stripped of all titles acquired due to the vast extent of the cheating. Trophies will not be reallocated. They will simply be retracted like other famous cheats. e.g. Lance Armstrong.

2) 150 point penalty. Whatever points they accumulate this season, will be deducted from their balance. This will result in them being relegated. Whenever they are promoted again, they will need to clear the remaining balance. e.g. if they can clear 85 points of their penalty this season, when they are promoted to the EPL next, they will still have another 65 points to clear from their ledger.
3) The EPL will need to conduct an audit to approximate how much each team has missed out on broadcast revenue etc for each of the cheating years plus an approximate value for each team which missed out on Champions League or were relegated from 18th position (maybe 50M for each of these as a rough guide). This will likely result in MC having to pay in excess of 750M to the EPL who will distribute based on an open, transparent calculation. This will need to be factored into all future FFP / SCR until cleared. Having a royal write a one off cheque does not qualify.
